Michigan’s families are still struggling with communication breakdowns between health care providers that result in a lack of care coordination and demonstrate the need for the AAP Medical Home Training. Coordinators are looking into the possibilities for the Family-to-Family Health Information Centers because it is so important that families have better access to information about available services. They also continue to work with the State Title V Program, including the Specialty Plans that serve children with special health care needs, and the states Early Intervention Systems.
